Some kind of utility hatch to access underground pipes bears a warning: "Danger - Permit required confined space - do not enter." Confined space with permitted entry typically means that the space is so tight that a spotter is necessary in case extrication becomes necessary. The space itself may not easily permit rescue efforts due to its size.
