The elevator is not malfunctioning. The car is just below the top floor in this picture. The access key switch inside the cab has been activated, allowing the access key switch on the top floor (where the boot is) to be set to the "down" position, disabling the door interlock and allowing the cab to descend so the top of the cab can be accessed. A fire alarm technician is about to climb on top of the cab to test the smoke detector in the hoistway.
